How to fill out a Georgia Sales Tax Certificate of Exemption (ST-5)

If you plan to sell products purchased from us in Georgia, we require that you fill out and e-sign the Georgia Sales Tax Certificate of Exemption form.

Here are some instructions on how to fill out the form correctly and prevent delays in processing your request:

• Be sure to enter the date on the top line.
• Purchaser's Name should be your registered business name.
• Sales Tax Number should be your registered business identification number.
• Type of business should describe your business activities.
• Address should be the registered address of your company.
• Sign the form along with your title.
• Telephone Number should be your business phone number.
• Email should be your business email address.
